• Maladie - medical prescription, health insurance card

• Accident - date of accident, claim number, name of insurer, medical prescription, health insurance card

Cancellation policy

We understand that life can be unpredictable, and sometimes you may need to cancel or reschedule your appointment. However, to ensure that we can accommodate all of our patients, we kindly ask for 24 hours’ notice if you need to cancel or reschedule. Sessions cancelled within 24 hrs will still be invoiced.
